The core difference is actually the depth and targeting of the effect - acupuncture can penetrate the skin directly to the deep meridian acupuncture points, and can even regulate the overall circulation of qi and blood in the organs. Massage mainly acts on the soft tissues from the surface to the middle layer such as muscles and fascia, focusing on loosening local strain and blockage.
I have been doing physical therapy for almost ten years, and I have encountered too many old pains that cannot be relieved by massage. Last month, a young girl who works in Internet design suffered from migraines for four years. She went for a head massage every week, which relieved her for two or three hours. As soon as the air conditioner was turned on or after a big need, the pain returned and she shed tears. I came here to diagnose a syndrome of blockage in the Shaoyang meridian. I found a few blocked points along the Sanjiao meridian and pricked them four or five times. Now the frequency of headache attacks has dropped by 80%. Even the sore throat I had when I got up early all year round is much better. This is why acupuncture can go to a depth that cannot be touched by massage.
Of course, this does not mean that massage is inferior to acupuncture. Think about it, if you have climbed a ten-kilometer mountain on the weekend and your legs are so sore that you can’t lift it, or if you have stayed up for three days and your neck and shoulders are so stiff that it is difficult to turn around, then go for a full-body relaxing massage. After the massage, most of the blood will be restored on the spot, so you don’t need to get an injection. After all, the soreness and swelling of acupuncture is quite difficult for people who are afraid of pain, and massage does not risk breaking the skin, so it is very friendly to people with sensitive skin or needle fainting.
To use an inappropriate analogy, our meridians are like the water supply pipes at home. Massage is equivalent to rubbing the soft scale on the outer wall of the water pipe. Once it is broken up, the water will flow smoothly and the comfort will come quickly. However, if there is hard scale blocked deep in the pipe, or the water pressure from the water plant is not enough, just rubbing the outer wall is useless. At this time, you have to rely on the thin probe of acupuncture to accurately locate the blocked position, and even adjust the water supply pressure at the source to solve the problem at the root.
There is still a lot of debate in the industry about the boundaries between the two. Some scholars believe that the so-called organ-regulating effect of acupuncture is largely a placebo effect, while the immediate relaxation and muscle tension relief of massage are the real effects that have been repeatedly verified. There is also a large amount of clinical data showing that acupuncture is effective in long-term improvement of functional conditions such as primary dysmenorrhea and chronic migraine, which is more than 30% higher than ordinary massage. There is no completely unified conclusion yet. In the end, it still depends on the specific symptoms and personal acceptance.
When I usually give patients a conditioning plan, I rarely use one method alone. For example, for patients in the acute stage of lumbar protrusion, I first use massage to loosen the spasm of the psoas muscles on the surface. Then when the needle is inserted, the pain is lower and the needle sensation is more easily transmitted. The effect is much better than using one method alone. There is no need to regard the two methods as either/or options.
