It is a normal physiological phenomenon for male testicles to be one high and one low. The asymmetric position of the testicles on both sides is mainly related to factors such as differences in scrotal structure, cremaster muscle contraction frequency, development process deviation, venous return pressure, daily activity habits and other factors.
1. Differences in scrotal structure:
There is a natural difference in fascia tension between the left and right sides of the scrotum. The left scrotum is usually about 1 cm looser than the right side. This structural feature makes the left testicles tend to be slightly lower than the right side. Anatomical statistics show that about 85% of men have left testicular ptosis, and the difference within 1-3 cm is within the normal range.
2. Cremaster muscle contraction frequency:
There is asymmetric movement of the cremaster muscle, which controls the rise and fall of the testicles, with the right side of the muscle contracting more frequently. When the ambient temperature changes or is stimulated, the asynchronous contraction of the cremaster muscles on both sides will temporarily amplify the testicular height difference. This dynamic adjustment helps to maintain an appropriate spermatogenic temperature.
3. Deviation in development process:
When the testicles descend from the abdominal cavity to the scrotum during the embryonic period, the right testicle usually completes migration 2-3 weeks earlier than the left. This timing difference may result in incompletely symmetrical development of the straps that secure the testicles, forming the anatomical basis for differences in position in adulthood.
4. Venous return pressure:
The left spermatic vein merges into the left renal vein at a right angle, and the blood return resistance is greater than the right side. Long-term venous hypertension may cause mild relaxation of the left scrotal tissue, which is more obvious in people who stand for a long time or in patients with varicocele, but no intervention is required when there is no pain or swelling.
3. Daily activity habits:
In right-handed people, the muscles in the right groin area are more developed and may slightly elevate the testicle on the same side. Certain special postures, such as long-term horse riding, cycling, etc., may also cause temporary position changes through continued pressure, which can usually be restored after changing the position.
It is recommended to choose breathable underwear to avoid local high temperatures, and wash with warm water regularly to keep the scrotum clean. If there is a sudden significant change in position accompanied by pain, induration, or volume change, you need to seek medical attention to investigate emergencies such as testicular torsion and epididymitis. Daily self-examination can be used to compare the position and texture of the testicles on both sides. Normal differences should not affect reproductive function and sex hormone secretion. Moderate exercise can help improve scrotal blood circulation.
