Semen after urination may be caused by prostatitis, seminal vesiculitis, urethral stones, residual sexual excitement, or abnormal urinary muscle contraction. It can be improved through anti-infective treatment, physical therapy, lifestyle adjustment, traditional Chinese medicine conditioning, and psychological intervention.
1. Anti-infection treatment:
Bacterial prostatitis or seminal vesiculitis are common causes, and sensitive antibiotics need to be selected based on pathogen detection results. Commonly used drugs include levofloxacin, azithromycin, etc., and the course of treatment usually lasts 2-4 weeks. When accompanied by symptoms of frequent urination and urgency, α-receptor blockers can be used to relieve urethral pressure.
2. Physical therapy:
Patients with chronic inflammation can use physical methods such as prostate massage and microwave heat therapy to promote local blood circulation. Deep hyperthermia 1-2 times a week can reduce glandular congestion, but is contraindicated during acute infection. Keeping the water temperature around 40°C during a sitz bath for 15 minutes each time can help relieve pelvic muscle tension.
3. Lifestyle adjustment:
Avoid sitting for long periods of time which may cause pressure on the perineum. Get up and move around for 5 minutes every hour. Reduce the intake of alcohol and spicy food, and drink no less than 2000 ml of water every day. Regular ejaculation helps to discharge accumulated prostatic fluid. It is recommended to maintain moderate sexual intercourse 2-3 times a week.
4. Traditional Chinese medicine conditioning:
For the damp-heat type, you can use Bazheng Powder plus or minus, and for those with weak kidney qi, Jinkui Shenqi Pills can be used. Chinese patent medicines such as Qianlieshutong Capsules have the effect of clearing away heat and promoting dampness, and can be taken continuously for one month. Acupuncture is performed on acupuncture points such as Guanyuan and Sanyinjiao three times a week to improve local Qi movement.
5. Psychological intervention:
Anxiety may aggravate abnormal contraction of the pelvic floor muscles and reduce nerve sensitivity through abdominal breathing training. Cognitive behavioral therapy can correct excessive focus on symptoms and recommends 15 minutes of mindfulness meditation every day. Partner participation in therapy can help relieve psychological stress.
Daily attention should be paid to observing whether there are warning symptoms such as hematospermia and ejaculation pain, and routine and ultrasound examinations of prostate fluid should be performed in a timely manner. Increase the intake of zinc-containing foods such as pumpkin seeds and tomatoes, and avoid activities such as cycling that continuously compress the perineum. Establish the habit of gently pressing the perineum after urination to help discharge residual semen. If symptoms persist for more than 2 weeks or worsen, the treatment plan needs to be reviewed and adjusted.
