Lower abdominal pain accompanied by testicular pain may be caused by testicular torsion, epididymitis, varicocele, urinary tract infection, or prostatitis. If symptoms persist or worsen, you need to seek medical attention promptly to investigate the cause.
1. Testicular torsion:
Testicular torsion is a urological emergency, often caused by rotation of the spermatic cord leading to obstruction of blood flow. The typical manifestation is sudden, severe pain in one side of the testicle that radiates to the lower abdomen, possibly accompanied by nausea and vomiting. Surgical reduction within 6 hours of onset of disease can avoid testicular necrosis, but delaying treatment may lead to impairment of reproductive function.
2. Epididymitis:
Inflammation of the epididymis caused by bacterial infection is common when a urinary tract infection spreads or a sexually transmitted disease occurs. The pain gradually spreads from the scrotum to the lower abdomen, accompanied by swelling and heat. Antibiotic treatment is required, and chronic and repeated attacks may affect sperm quality.
3. Varicocele:
Poor spermatic vein return leads to a feeling of swelling in the scrotum, and pain radiates to the lower abdomen after standing for a long time. There are multiple lesions on the left side, which may cause the temperature of the testicles to increase and affect the spermatogenesis function. If the disease is moderate or above, high spermatic vein ligation under a microscope should be considered.
4. Urinary tract infection:
When the bladder or urethra is infected, the inflammatory stimulation can radiate to the perineum, manifesting as dull pain in the lower abdomen combined with a burning sensation during urination. Escherichia coli is a common pathogenic bacteria that may cause pyelonephritis if not treated promptly.
5. Prostatitis:
Chronic pelvic pain syndrome is more common in young and middle-aged men, manifesting as dull pain in the lower abdomen and testicles, which may be accompanied by frequent urination and urgency. The cause is related to lifestyles such as sitting for a long time and drinking alcohol, and it needs to be confirmed by prostate fluid examination.
It is recommended to avoid riding bicycles or sitting for long periods of time, and taking a daily warm water sitz bath for 15 minutes can relieve local discomfort. Pay attention to supplementing zinc in your diet such as oysters and nuts, and reduce the intake of spicy and irritating foods. During acute pain, sexual intercourse should be suspended and loose-fitting cotton underwear should be worn. If the pain lasts for more than 24 hours or is accompanied by fever, hematuria and other symptoms, you need to immediately go to the urology department to check for emergency treatment. Regular physical examinations, including scrotal ultrasound and prostate-specific antigen testing, can help detect potential lesions early.
